Single Page Entry Forms!

No more filling out multiple cards as before.

Not all shows will be doing the single page entries
The June 17th and June 24th shows WILL be using the Single Page Entries

This is a trial program to see how it works out for our exhibitors and show personnel
Your co-operation will be greatly appreciated!

2012 NEOHA Class Entry Form

The convenience of the Single Page Entry Forms will make the work
of the show personnel a little harder. Each entry will need to be transferred
to a class sheet. The class sheet will be used for announcing and marking
the placings of the class.

To make this job a bit easier, we would like to pre-enter some of the information
into our class sheets. Please use the form below to give us some information about you,
your horse, and your classes you maybe entering in. By entering this info, you are under
no obligation to enter any of your possible classes. When you fill out your entry form, and give
it to show personnel, we will be able to simply check a box on the class form that you're entered.

Your information will be pre-printed on our individual class forms!

To be eligible for year end awards, exhibitor must be a Dusty Boots member and the horse must be nominated.

Dusty Boots is going GREEN. We are recycling slightly used trophys.
We will be using them for the 10 & under walk trot and leadline classes.
